Lakeview Pioneer Lodge Inc

Lakeview Pioneer Lodge is an affiliate organization of the Saskatchewan Health Authority. Today we provide care for 45 individuals who are no longer able to remain independent in their own homes. Admission into our home is accessed through home care. Home care assesses potential residents and prioritizes admissions. Our local home care office is located at the Wakaw CEC and may be reached by calling (306) 233-4611.

Documents we will  require......

  1. Power of Attorney ( if applicable)  
  2.  Copies Income tax  - Previous 2 years  
  3.  Living Will  ( if applicable)  
  4. Appropriate agreements printed and completed from above links.  
  5. Voided check
